Adult, Child, Infant CPR

To reserve a space in the Pediatric CPR Class, please complete this application and the first non-refundable tuition payment of $50.00 (Initial Certification), $40.00 (Renewal Course). If this is a renewal course you must show proof of certification with a valid CPR card. We  accept MC/VISA/AX/Discover.

If payment is to be made by your employer or other agency, we require a purchase order or letter from your employer confirming and authorizing such arrangement. We will need this confirmation at least one week prior to the start date of the first class.*

Tuition Refund Policy
If for any reason you would need to cancel your registration, we require a minimum of a 7 day notice to receive a full refund otherwise you are responsible for and will be billed the course total. There is no fee to reschedule to a different Con Ed class provided a 7 day notice is given.
