Emergency Medical Technician-Basic Program Application

To reserve a space in the EMT-Basic Program, please complete this application and the first non-refundable tuition payment of $215.00. We  accept MC/VISA/AX/Discover. The remaining tuition amount is due prior to the first class meeting. A payment plan option is available, please contact us for information.

If payment is to be made by your employer or other agency, we require a purchase order or letter from your employer confirming and authorizing such arrangement. We will need this confirmation at least one week prior to the start date of the first class.*

Registration Requirements

  1. Completed registration form below.
  2. Payment for the course (First payment of $215.00 or full tuition) paid online.
  3. A course syllabus, information packet, and textbooks will be handed out the first night of class.
  4. You do not have to be 18 years of age to participate in this program, although you MUST be at least 18 years of age to take the State certification Examinations.

Failure to meet all requirements set forth by the Commonwealth of Massachusetts Office of Emergency Medical Services could result in the postponement of your State Exam.

Tuition Refund Policy
If tuition is paid in full prior to the start date of your program depending on your session choice and you need to withdraw prior to this date you will be refunded the tuition amount minus the non-refundable first payment of $215.00.

  • Withdrawal after the first class but before the second class begins a 50% reimbursement minus the non-refundable first payment of $215.00.
  • Withdrawal after the second class but before the third class begins, a 20% reimbursement minus the non-refundable first payment of $215.00.
  • No reimbursements will be issued there after.

Note: As part of this program, you will also be certified at the required Healthcare Provider CPR level. If you are currently certified at the American Heart Association Healthcare Provider CPR or American Red Cross Professional Rescuer CPR level, your card must be current throughout the EMT-Basic Program.
